Full-time students and those 65 years of age and over are excluded. Variables that do not change over time (ie, gender and place of birth), that are little or slow changing (eg, ethnicity and highest qualification) or that could be impacted on by health changes (ie, studying status and years in paid employment) are excluded from these models. Significant and insignificant variables or variable categories are kept in for completeness.

2. The relationship between changes in self-rated health and participation was modelled.
